Theresa Madaline, MD, clinical director of Infectious Diseases Services, Jack D. Weiler Hospital and Riganni Urrely, LPN, nurse coordinator, Outpatient Antibiotic Therapy Program, discuss their study that showed Montefiore’s Transitions-of-Care Outpatient Parenteral Antibiotic Therapy program, which features streamlined communication with patients and bilingual providers, reduced emergency department readmissions for patients who receive IV antibiotics outside of the hospital.
